Liz good advice. A planted 220g tank is hard to keep up with bio load.
This doesn't look like any HITH I've seen, although I have seen the pimples explode like this and the result was not good. I would still recommend isolating and treating with salt and/or antibiotics.

Metro is pretty mild and will work in conjunction with most other antibiotics so I don't see any harm in this. And, I wasn't disputing the HITH theory, just that it's not what I've seen in the past. I'm hoping this is not aeromonas or a flesh eating bacteria. If it is then isolating is even more critical.

I don't think you had HITH since it lasted only 2 days, mostly likely some water quality issues. HITH are usually around the eye area and sometimes on the lateral lines. Stress can bring out the pimples along with other hidden diseases that we can't see physically.

The HITH goes in stages:
1. Pimple near head and eye
area
2. Pimple pops, thread stringy white mucus(Vilo's starting second stage)
3. All white stringy mucus gone leaving a hole to heal up most of the time but may come back frequently if not treated.
With all said I think Vilo has a secondary problem besides the pimples. Samples of the skin around the infected area may be necessary.

I can't put my finger on the cause but I will bet something from your 'domestic discus' is attacking your wilds as they often have not developed resistance to many of the issues that can lurk in our aquariums . Do you know your pH /GH /KH ? They may not be important but some wild fish are very particular to water parameters beyond nutrient levels ..
Vilo emailed me this morning or late last night. The fish with the weird symptom died. I can't imagine what could have killed it so fast. I've never in 20 years in Discus seen anything pathogen that kills so fast.

Sorry to hear! But that was an ugly looking disease. He’s now got to keep an eye on his main tank.
